Dear fellow club members,

The web link to access the insurance information is available on the CRW's
member <http://www.crwoodturner.com/members.shtml> website page. Click
on the Member Insurance Info
<http://www.crwoodturner.com/insurance/CRW_insurance_info_sheet.html> link
to obtain this useful informational page.

Of particular note is the Brief directions
<http://www.crwoodturner.com/insurance/CRW_Accident_Ins_process_brief.html>
page. This page provides the simple process to follow:

. when 2 or more woodturners are engaged in either a Mentoring
Workshop Activity, such as working at someone's home shop, or

. when 2 or more woodturners are engaged in an External Activity,
such as attending and participating in an art fair demonstration, demo at
nursing home, Norskedalen, etc.

Please note, according to the provisions of the accident medical insurance
policy (through Gerber Life Insurance), that all woodworking/woodturning
activities must be directly supervised by a "person authorized". As paid up
CRW members, we are all considered as a "person authorized ". There is
also coverage for members traveling in a group of 2 or more when "traveling
directly (uninterruptedly)" to and from an event.

The club's adopted process should be followed if members wish to take
advantage of the provisions of accident plan. It's important we cross the
t's and dot the i's when dealing with any potential insurance claim.

Briefly:

1-Prior engaging in a Mentoring Workshop or External Activity, notify our
current CRW secretary Ginny Wintersteen, either by email or sending a note
via snail mail of your scheduled event. Alternatively, the notification
could go to any club officer. Please list the event name, the sponsoring
member's name, the location and the date of the activity. You may use the
Mentoring

5-10-15.pdf> Workshop Attendee or External Activity
<http://www.crwoodturner.com/insurance/CRW_External_Activity_workshop_ins_fo
rm_final5-10-15.pdf> Attendee form, depending on the activity. Examples for
filling out the form are provided within each form.
This is an important step, as the date/time stamp on the email or postal
date may be needed to verify the club membership is following the
provisions of the policy as applied to a "scheduled meeting/activity".

2- While at the actual event, fill out the actual Mentoring Workshop or
External Activity attendee form. CRW members will sign in by printing their
name and placing their signature in the spaces provided, and providing an X
in the proper place to indicate CRW and or AAW membership, or if you are a
guest. Please know that only active CRW members are entitled to the
provisions of the accident insurance policy should an injury be sustained.

3. Upon completion of the activity, send the appropriate completed form to
Ginny, either by email or snail mail. Alternatively, the completed form
could go to any CRW board officer. Please note, the forms will be archived
as may be needed to verify your attendance at the activity to the insurance
company, should you be injured and file a claim. Any claim should be filed
with the Gerber Life Insurance Company within 30 days of the injury.

For more information on the specifics of the CRW's accident insurance
program process, please read the Document of Understanding
<http://www.crwoodturner.com/insurance/CRW_insurance_understanding_document_
final.pdf> . The document breaks down some of the complexity of the actual
insurance policy, provides common sense definitions to the wording in the
policy, and specifies how to file a claim should that be needed.

Please note, the accident medical insurance covers all medical costs not
covered by your primary health insurance up to the policy limits. If you
have no primary health insurance, this policy will pay all costs up to the
policy limit. The accident insurance has a deductible of $100.00. No
co-pays or co-insurance applies. Members cannot obtain payment from multiple
insurance companies for medical care. If your health insurance has a
deductible, the CRW policy will pay the deductible up to the limits
specified in the policy.

Please note, to be eligible for the provisions of accident medical insurance
program, you must be a paid up member of the CRW. Non members and the
general public are not covered for medical expenses under the Accident
Insurance policy.

The complete accident policy is or will be on file in the CRW's library and
eventually on the CRW's website.

Injuries sustained by the general public during a demonstration are covered
the CRW's liability insurance policy(s) should a lawsuit occur.

And finally, thanks to Ken Grunke for his help with this project by putting
insurance information on the CRW website.

Respectfully submitted, Bob Raasch

CRW member
